    I , a self-employed contractor, confirm that I have read the About Time Limited COVID-19 Contractor protocol and understand the content therein.
    I oblige to comply with all the rules and procedures as outlined in the Document.
    I will observe social distancing and the measures as outlined in the Contractor’s Responsibility section, at all times.
    I am aware of the signs & symptoms of COVID-19, and will monitor my own wellbeing, self- isolate at home and contact GP promptly for further advice if I display any signs of symptoms. I will remain at home and not attend any of my clients until all symptoms are have cleared following self-isolation.
    I will complete and return the pre-return to work form before attending any of my clients and will inform About Time Limited, if there are any other circumstances relating to COVID-19, not included in the form, which may need to be disclosed to my clients to allow safe return to their homes and/or other premises.
    I will complete any temperature testing as implemented by my clients or About Time Limited and inline with the Public Health advice
